A simple, four-step process.
We designed the report form to be fast and not overwhelming. The first thing we ask is "what happened?" — everything else is optional or quick to fill in.
Pick the category that fits — trucking, workplace, financial abuse, medical, veterans, religious, government, or other. If your situation isn't listed, choose Other and describe it in your own words.
Tell us what happened. Include the company or person involved, location, and time period. Upload evidence (images, PDFs, documents) if you have it. Choose to share your identity or stay anonymous.
Immediately after submission, you receive a unique Case ID like STI-2026-000123. If you provided email, we send it to you as well.
Save your Case ID. It's the permanent reference number for your report. If you provided contact information, we may follow up if more information is needed.
